Change Grow Live
Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives.
Our core values are ‘Be open, be compassionate and be bold’ and our teams apply these daily, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.
As our Partnership Development and Training Lead, you will play a vital role in ensuring that the people we support receive meaningful, robust, and holistic care.
You will help strengthen multi‑agency collaboration across the local health and social care system, ensuring that partners and professionals have the knowledge, pathways, and support they need to work with individuals experiencing drug and alcohol challenges.

A key part of your role will be building and expanding our partnership networks. This includes identifying gaps, creating new links, and ensuring partners are informed about the treatment we provide, emerging trends, drug alerts, and best practice guidance.
You will deliver training and upskilling across statutory, non‑statutory, and third‑sector organisations, helping to embed a consistent and informed approach across the community.
You will work closely with healthcare settings including Primary Care Networks, GP Shared Care, Mental Health services, and Hospitals. Acting as a key point of contact, you will strengthen communication, develop co‑produced pathways, and support joint working that improves access to treatment.
You will also contribute to service development by supporting audits, reviews, and continuous improvement activities, ensuring the service maintains high professional standards and positive outcomes.
This is a dynamic and varied role that offers you the opportunity to make a system‑wide impact through partnership working, workforce development, and community education.
