Duties

- Manage complex cases involving critical care, trauma, infectious diseases, and behavioral health conditions.
- Collaborate with multidisciplinary teams to develop and implement individualized discharge planning and treatment strategies.
- Document medical records accurately in compliance with HIPAA regulations and maintain detailed case management reports.
- Perform diagnostic evaluations and coordinate diagnostic testing procedures to support clinical decision-making.
- Monitor patient progress through continuous assessment and adjust care plans accordingly to optimize outcomes.
- Participate in utilization management activities by evaluating patient needs for hospital admission or outpatient services.
- Support intake processes for new patients, ensuring comprehensive medical histories and current health status are recorded.
- Assist with medical records management, ensuring confidentiality and compliance with healthcare standards.
- Contribute to hospital quality initiatives by adhering to infection control protocols and safety procedures.

- Strong knowledge of hospital medicine, emergency medicine, and acute care practices.

- Familiarity with medical terminology, physiology principles, and diagnostic evaluation techniques.
