Community Health of Central Washington (CHCW) is seeking a Family Medicine or Internal Medicine Physician, or Geriatrician, to provide primary care services to senior and residential care clients. This role involves serving as a Medical Director for skilled nursing facilities, integrating clinical and administrative duties.

- Integrated community health center
- Full spectrum primary care, pediatric, senior residential care, behavioral and addiction care, and dental services
- Serves Yakima and Kittitas Counties at multiple sites
- Senior and Residential Care program (SR Care)
- Expected volume: 14 patients assigned per day, typically at 1 facility per day
- Modern Tech: OCHIN EPIC
Responsibilities
- Provide primary care services to SR Care clients at participating LTC facilities
- Serve as the individual SNF's Medical Director
- Integration and management of care to include inpatient admitting, clinical, and admin services
- Advocacy for patients and providers
- Participate in administration round tables for input with strategic planning
- Share passion with residents and medical students
Compensation
- Base Salary: $237,500-$247,500
- Additional financial add-ons include a quality/engagement incentive
- $10,000 fellowship bonus (if applicable)
- $4,000 per SNF oversight (8 contracted SNF facilities shared)

Requirements
- MD/DO degree or IMG equivalent
- Completion of an accredited US FM or IM Residency
- Preference of geriatric medicine certification, or FM/IM with added qualifications in geriatrics
- Certificate of Medical Director for PALTC (AMDA) (must obtain within 3 years from start date)
- WA License or the ability to obtain a WA license
- DEA certificate
- BE/BC required
- Approved for credentialing with Medicare, Medicaid, and health plans
