What You'll Do

• Manage and mentor a team of home health nurses and therapists, fostering excellence in care delivery.
• Ensure all home health services align with care plans and regulatory standards.
• Oversee care quality in homes, nursing facilities, and board & care settings.
• Optimize workflows and policies to enhance efficiency and participant outcomes.
• Collaborate with interdisciplinary teams to address complex clinical needs.

What We're Looking For

• Active California RN License (required).
• 2+ years of home health experience, preferably with seniors or frail populations.
• Strong clinical judgment and leadership skills.
• Passion for improving care for vulnerable seniors.
