Clinical Magnet, a division of ICON Medical Network, is seeking an experienced Director of Case Management for a full-time leadership opportunity in Yuma, AZ. This position reports directly to the CEO and serves as a key member of the hospital leadership team.
The Director will oversee daily operations of the Case Management department while supporting exceptional patient outcomes, regulatory compliance, and efficient care transitions.

Director of Case Management Responsibilities
- Oversee daily operations of the Case Management department
- Lead utilization review, discharge planning, and care coordination activities
- Ensure compliance with hospital policies, regulatory requirements, and quality standards
- Collaborate with physicians, nursing leadership, therapy teams, and hospital administration
- Mentor, develop, and support Case Management staff members
- Monitor departmental performance metrics and implement process improvements
- Assist with patient caseloads as needed during periods of low census or staffing shortages
- Promote effective patient flow and successful transitions of care
- Support hospital initiatives focused on quality outcomes, patient satisfaction, and operational efficiency
Leadership Oversight
- Directly supervise a multidisciplinary Case Management team including:
- 3 RN Case Managers
- 1 LPN
- 3 Medical Social Workers
- 1 Licensed Social Worker
- Lead a department supporting a 61-bed inpatient rehabilitation hospital with an average daily census of approximately 56 patients
- Foster a culture of collaboration, accountability, and patient-centered care
Qualifications
- Registered Nurse (RN) or Licensed Social Worker required
- Bachelor's degree required (BSN required for RN candidates)
- Master's degree preferred for non-nursing healthcare professionals
- Minimum of 3 years of hospital-based Case Management experience required
- Strong experience with both Utilization Review and Discharge Planning required
- Acute care and/or inpatient rehabilitation experience strongly preferred
- Active professional license required if applicable to discipline and state requirements
- CCM or ACM certification preferred; required within the first year of employment if not currently certified (employer-sponsored certification support available)
- Demonstrated leadership experience within a hospital setting preferred
- Strong communication, organizational, and interdisciplinary collaboration skills
Compensation & Benefits
- Salary range: $115,000 – $120,000 annually
