Integrated Health Management Specialist

Job Overview

As an Integrated Health Management Specialist, you will play a pivotal role in transforming care delivery and clinical operations to enhance efficiency, optimize utilization, and elevate the performance of healthcare providers and payers.

Key Responsibilities

• Design and implement innovative preventive, chronic, and complex care programs aimed at closing care gaps.
• Lead initiatives for care coordination and transitions of care across payer, provider, and community settings.
• Support the development of value-based care strategies, efficient operating models, and performance management processes.

Qualifications

• At least 5 years of consulting experience with healthcare payers and/or provider clients.
• A minimum of 2 years of experience in population health, care management, or value-based care programs.
• Bachelor's Degree required.
• Experience with population health platforms or care management technologies.
• Familiarity with Medicaid, Medicare Advantage, or ACO operating models.
• A background in health equity, community-based care, or behavioral health integration is preferred.
• Solid understanding of longitudinal and cross-sector care models.
