You will collaborate with physicians, nurses, and other healthcare professionals to improve patient care, participate in high-risk rounds with a multi-disciplinary team to discuss patient progress and adjust treatment plans, and provide excellent patient counseling to communicate complex medical information to patients and healthcare providers.
Participation in quality improvement initiatives aimed at enhancing medication safety and efficacy is expected, including post-discharge medication reconciliation for high-risk transition of care patients.
Documentation is an important part of the job: document all activity within clinic EMR and other required pharmacy or clinical management systems, support CMS Stars and HEDIS quality measures for successful clinic performance, and maintain accurate documentation of clinical interventions and patient care activities.
You will also conduct in-service education sessions for healthcare staff, develop and share educational materials related to pharmacotherapy, stay current with advancements in pharmacotherapy and clinical guidelines, and provide consultation to healthcare providers on complex medication management issues.
You will respond to additional requests from the Primary Care Organization, providing support within the pharmacist's defined scope of practice to advance organizational goals. Address and implement targeted initiatives or priorities as directed by the National Director or Associate Directors of Pharmacy, according to best practices and regulatory standards.
Active and unrestricted Pharmacist license. Willingness to become licensed in additional states as needed for market support. Pharm.D. or other advanced pharmacy degree. 2+ years' experience as a practicing pharmacist. Intermediate level or higher competency with Microsoft Office programs such as Word, Excel and Power. Point.
Preferred qualifications include an active and unrestricted pharmacist license in North Carolina and South Carolina, 5+ years' experience as a practicing pharmacist, 2+ years working in an ambulatory care/primary practice setting, clinical residency in ambulatory care or similar discipline, board certification in geriatric pharmacy or pharmacotherapy, familiarity with electronic medical record (EMR) systems, experience implementing solutions for improved efficiencies and outcomes, and multilingual ability (preferably English/Spanish).
This is a remote Work at Home role. To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested.
Satellite, cellular and microwave connection can be used only if approved by leadership. Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense. Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.
Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours 40.
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$126,300 - $173,700 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
