Registered Staff Nurse (2 yrs IV Therapy & Wellness exp req)

FORM Wellness is expanding. We are launching IV Therapy inside our luxury wellness studio and are looking for an experienced Registered Nurse to help bring this offering to life.

This role is flexible by design — ideal for a Nurse who wants to start part-time or on-call, with the opportunity to grow alongside us as the IV program expands. If you prefer to remain part-time or on-call long-term, that is completely welcome as well.

FORM is not a clinical, hospital type environment. Our studio is upbeat, elevated, and community-driven. We know our patients by name, we take our time, and we pride ourselves on providing exceptional care in a calm, refined setting.

Role Overview

The Nurse will be responsible for administering IV therapies and supporting the safe, professional growth of our IV program. This role requires impeccable IV skills, strong clinical judgment, and an excellent bedside manner.

You will work independently while collaborating closely with the FORM team to ensure every patient feels comfortable, cared for, and confident throughout their experience.

Key Responsibilities

• Administer IV hydration and nutrient therapies with precision and confidence
• Perform patient assessments and medical screenings prior to IV administration
• Ensure patient safety, comfort, and education throughout the IV experience
• Maintain accurate documentation and adherence to all medical protocols
• Uphold FORM’s high standards for patient experience and professionalism
• Collaborate on the growth and evolution of the IV program as services expand
• Represent FORM Wellness with warmth, integrity, and attention to detail

Who We’re Looking For

• Licensed Nurse in the state of Tennessee
• Exceptional IV skills — confident, efficient, and patient-focused
• Experience in IV therapy, infusion services, wellness, or related settings preferred
• Calm, polished bedside manner with strong communication skills
• Aligned with a community-based, relationship-driven approach to care
• Open to growth, but equally comfortable remaining part-time or on-call

Job Types: Full-time, Part-time

Pay: $27.00 - $35.00 per hour
