Edmonton Southside Primary Care Network – Primary Care Registered Nurse
The Edmonton Southside Primary Care Network was the first PCN in Alberta. Over the past 17 years, the ESPCN has grown to become the largest PCN in Edmonton and consists of a team of health care professionals who are passionate about creating healthier communities. We are looking for a permanent, part-time (0.80) Nurse. The Primary Care Nurse will provide comprehensive nursing services and care to patients within the Primary Care Network. The Primary Care Nurse will assess, plan, implement and evaluate nursing care plans to patients of all ages.
The Primary Care Nurse will be an integrated team member working to their full scope of practice, including performing restricted activities as outlined in the Health Professional Act within the primary care setting.
The Primary Care Registered Nurse (PCRN) will be an integrated team member at the ESPCN Central Office.
The PCNRN will engage in a wide range of activities to support comprehensive patient care, including consultation and interventions for patients referred to the Central Office, program development, review, and delivery (in-person and virtual), nursing support for centrally delivered programs and services, and care coordination to manage patients referred to the central office.
The Primary Care Registered Nurse – Central Office will also provide occasional PCRN coverage in ESPCN clinics and engage physicians who do not have a co-located PCRN to establish a PCRN role within their clinic.
The Primary Care Registered Nurse will work with the family physicians in the community, other health professionals and local and regional programming in a multidisciplinary environment to ensure a high quality of evidence-based health care to the practice population
