About Us

We are a group of healthcare providers currently practicing throughout the Dallas–Fort Worth (DFW) area. At present, our team includes 6 Nurse Practitioners providing care across multiple Nursing Facilities, and we are continuing to grow. As we expand our services, we are looking to add dedicated Nurse Practitioners to our team.

Our practice specializes in the management of patients requiring Infectious Disease services in Skilled Nursing Facilities (SNFs). We work closely with facility staff to provide high-quality, patient-centered care and ensure timely follow-up for medically complex patients.

Position Summary

The Nurse Practitioner will make scheduled visits to our contracted Skilled Nursing Facilities to evaluate and manage patients under the clinical supervision and collaboration of our experienced provider. The role focuses primarily on patients requiring Infectious Disease consultation and follow-up care.

This position offers flexible scheduling, excellent compensation on a per-encounter basis, and minimal administrative burden.

Responsibilities

• Conduct routine patient visits at assigned Skilled Nursing Facilities.
• Perform comprehensive patient assessments and follow-up evaluations.
• Review laboratory results, cultures, imaging, and other diagnostic studies.
• Develop and implement treatment plans in collaboration with the supervising provider.
• Monitor patients receiving antimicrobial therapy and other Infectious Disease-related treatments.
• Coordinate care with facility staff, nurses, therapists, and primary care providers.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Communicate significant clinical findings to the supervising provider.
• Participate in quality patient care while adhering to all applicable regulations and clinical standards.

Qualifications

• Active Nurse Practitioner license in the applicable state.
• National NP certification.
• DEA registration (preferred or required based on state regulations).
• Current BLS certification.
• Experience in Skilled Nursing Facilities, Internal Medicine, Hospital Medicine, Geriatrics, or Infectious Disease is preferred but not required.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating with the supervising provider.
• Reliable transportation for travel between assigned facilities.

Compensation & Benefits
