What you’ll do

• Assist residents with activities of daily living such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary
• Monitor patient vital signs and observe changes in health or behavior to report to healthcare professionals promptly
• Prepare nutritious meals tailored to individual dietary needs and assist residents during meal times to ensure proper nutrition and hydration
• Support medication administration following HIPAA guidelines and care plans established by healthcare providers
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and laundry
• Document patient observations accurately in compliance with privacy regulations like HIPAA
• Provide emotional support and companionship to promote mental well-being and social engagement

Responsibilities

• Deliver direct support to residents by assisting with mobility, transfers, and positioning using appropriate equipment such as Hoyer lifts or resident’s (personal equipment)
• Observe and record changes in residents’ condition or behavior related to dementia, Alzheimer’s, or other cognitive impairments for ongoing care planning
• Follow established care plans meticulously to ensure consistent quality of care tailored to each individual’s needs
• Assist with light housekeeping duties including cleaning rooms, changing linens, and maintaining a hygienic environment
• Support residents during outings or community activities to foster social interaction and community engagement
• Adhere strictly to safety protocols including CPR/First Aid certification requirements and emergency procedures for hospice or home health situations
• Collaborate effectively with healthcare teams, social workers, family members, and other caregivers to coordinate comprehensive care

Requirements

• Prior experience working in assisted living facilities, nursing homes, group homes or home & community care environments preferred
• Knowledge of patient observation techniques and ability to monitor vital signs accurately
• Certification in CPR/First Aid is required; additional certifications such as HIPAA training are advantageous
• Experience assisting with ADLs (Activities of Daily Living) including bathing, dressing, grooming, meal preparation, and medication management
• Ability to lift heavy objects safely when assisting residents with mobility or transfers using Hoyer lifts or similar equipment
• Strong communication skills coupled with a compassionate attitude towards individuals with disabilities or cognitive impairments such as dementia or autism spectrum disorder
• Willingness to work flexible hours including evenings or weekends as needed for resident care needs
