The post holder is responsible for the assessment, planning, implementation and evaluation of high‑quality nursing care within a multi‑disciplinary environment, working in accordance with NMC: The Code, the Gold Standards Framework, and the values of St Bartholomew’s Court.
The post holder will undertake the full range of nursing duties outlined in this job description and will be expected to take charge of the unit when required, ensuring the safe and effective delivery of care to residents.
The role supports the continuity of care for service users throughout their journey within the service. Although primarily based within the inpatient environment, the post holder will work flexibly across both units to meet service needs.

Clinical Responsibilities
Assessment & Care Planning - Assess needs, plan, implement and
evaluate individualised programmes of care in line with NMC The Code.
Named Nurse Role - Act as Named Nurse when allocated, ensuring continuity and quality of care.
Clinical Standards - Ensure nursing practice complies with organisational policies, evidence-based standards, and regulatory requirements.
Monitoring & Evaluation - Participate in ongoing audit, review and evaluation of nursing practice.
Care Pathways - Support the development and use of integrated care pathways and appropriate clinical assessments.
Admissions - Contribute to pre-admission assessments and complete required reports.
Therapeutic Relationships - Build effective, compassionate relationships with residents and families.
Supervision - Participate in clinical, managerial and case supervision.
Safe Environment - Promote a safe, dignified environment respecting privacy, culture and choice.
Health Promotion - Incorporate health education into daily practice.
Record Keeping - Maintain accurate, timely records in line with NMC and organisational standards.
Medicines Management - Administer and manage medicines safely in accordance with policy and NMC guidance.
Teamwork - Work collaboratively with the MDT and external agencies.
Managerial Responsibilities
Operational Support - Support the implementation of clinical and operational procedures.
Induction & Mentorship - Assist with induction; act as mentor or
preceptor to junior staff and students.
Coordination of Care - Assist in coordinating care delivery across the
24-hour period.
Clinical Governance - Promote high-quality, evidence-based care and
contribute to audits and quality assurance.
Legal Compliance - Ensure care is delivered in line with the Mental Health Act, Mental Capacity Act and associated Codes of Practice.
Communication - Maintain effective communication within the team.
Complaints & Incidents - Assist in resolving complaints and support incident investigations.
Equipment & Safety - Support maintenance of equipment and
adherence to Health & Safety, COSHH and Medical Devices guidance.
Staff Support - Support the monitoring and development of junior staff performance.
Delegated Duties - Undertake tasks delegated by the Matron/Deputy Matron.

Safeguarding Practice - Adhere to all safeguarding policies and act immediately on concerns.
Accurate Records - Maintain clear, factual safeguarding documentation.
Confidentiality - Protect confidentiality at all times.
