Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Clinic Care Coordinator/ PSA.

Job Responsibilities

• Assisting in coordinating and providing patient care activities.
• To use of IT systems to track, perform and document established clinical indicators and care given to patient.
• Perform basic parameter measurements, close preventive screening and vaccination gaps in relation to national programmes/initiatives.
• Provide basic health education to improve patients’ knowledge on lifestyle management, social and physical activities through social prescribing
